When we lose one or more teeth, a process of loss of volume and density of the alveolar bone begins. During the first year, there is a 25% loss in width followed by a loss in height. Once all the alveolar bone is lost, the basal bone begins to decrease gradually, thus causing significant functional and aesthetic problems. The placement of a dental implant acts as if it were a natural tooth, thus stimulating the bone to maintain its volume and density.
Dental implants are a fixed, long-lasting solution for replacing missing teeth. The most important concept within oral implantology is that of osseointegration. The discovery of Ooseointegration, serendipitously, revolutionized the field of dental implantology; the fact that a material biologically fused with bone was one of the most significant scientific advancements in Dentistry since the late 1970s. The dental implant consists of a Titanium screw that is accepted by the body as its own. The placement of the dental implant requires a period of osseointegration of about 2-3 months from the moment it is inserted into the bone until the definitive crown is placed. Nowadays, significant advances in surgical techniques and dental implant technology make it so that in very few cases is it not possible to place dental implants. Nonetheless, it is important for the patient to have good control over any potential pathologies they may have and habits; for example, patients who have received radiation, uncontrolled diabetes, smokers, periodontal problems, among others; in addition to good oral hygiene control.

Treatment depends on how many teeth are missing and the bone available:
One implant with a crown on top. The neighbouring teeth don't need to be filed down, as with a bridge.
A fixed bridge on implants: a few implants replace several teeth in a row.
A fixed screw-retained full prosthesis or a removable overdenture held by implants, which doesn't move when you eat or talk.
After the placement of the dental implant, a period of osseointegration of about 2-3 months is necessary from the moment it is inserted into the bone until the definitive crown is placed. During this process, the implant may be totally buried beneath the gum, which implies two surgical interventions, or with a protection screw emerging from the implant and protruding from the gum or even with a dental crown/prosthesis, which implies in both cases a single surgical intervention. In the case of performing an immediate load of the dental implant, not only does it include a single surgical stage but also, it loads the implant with a restoration/ provisional crown the same day as the intervention or shortly thereafter. This process has its risks since the interface between the implant and bone is weaker and presents a higher risk after passing between 3 and 6 weeks after the placement of the implants, because during this period the bone is less mineralized and disorganized. Nonetheless, immediate loading or aesthetics helps us achieve good remodelling of the soft tissues surrounding the implant and neighbouring teeth, as this is the key to success in anterior aesthetics.
The placement of dental implants after tooth extraction has advantages and disadvantages. Immediate implants combine the implant osseointegration period with the healing of the alveolar process after extraction, thus reducing the treatment time for the patient. On the other hand, the limitations of immediate implants are the difficulty of placing them in the ideal position, the lack of bone covering the implant surface, thus requiring the need for bone grafts, and the inability at times to completely cover the defect and the protective barrier with soft tissue. Therefore, when possible, the implant can be placed immediately after tooth extraction, in which cases the use of bone grafts may be required. If immediate implantation is not possible, it will be necessary to wait between 3 and 4 months, for good healing of the alveolar process and subsequent placement of the dental implant.
The aim of bone regeneration treatment is to reconstruct defects in the alveolar bone prior to or at the time of placement of dental implants. In treatment with bone grafts, a barrier is used to isolate and create a protected space for good osseointegration between the biomaterial and the alveolar process, this barrier is called a membrane. The use of bone graft allows us to create new bone, bone that has been lost. Performing a premature load of dental implants can worsen the quality of the grafted bone, hence the limitations of immediate load implants.
